29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a- 30 North Poplar Avenue, Maple Shade NJ: On or about 04/27/17, company workers performing sheathing installation and other related activities were exposed to fall hazards in excess of 14 feet the ground surface below when working from a metal platform that was resting on the forks of a Gehl Type D all-terrain fork truck. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
