29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and pinch points: a) Production Floor; On or about April 28, 2017 and at times prior thereto the employer exposed employees to amputation and crush hazards in that employees were required to operate the MT Carlson Door Press Machine which had unguarded pinch points. b) Production Floor; On or about April 28, 2017 and at times prior thereto the employer exposed employees to amputation and crush hazards in that employees were required to operate the MMTX Carlson Door Press Machine which had unguarded pinch points.
