29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a) Throughout the facility - During the period between May 1, 2017 and June 1, 2017 employee(s) were exposed to potentially hazardous energy sources when performing service and/or maintenance on the Miyano Bar Lathe (employer asset #004) as the documented energy control procedure (Lockout/Tagout) developed by the employer and located on machine was deficient in that it did not;  (1) Outline the scope of the procedure,  (2) Describe the intended purpose of the procedure,  (3) Describe the means to enforce compliance with the procedure, and  (4) Identify the magnitude(s) of energy for all hazardous energy sources.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
