29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a)    At the Garden City warehouse: On or about April 27, 2017, and times prior thereto, the employer did not ensure that eye/face protection was worn by forklift operators exchanging 33-lb. propane cylinders, exposing employees to hazards not limited to facial burns.    Distribution Services International, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(1), which was contained in OSHA inspection number 951934, citation number 1, item number 2, and was affirmed as a final order on May 22, 2014, with respect to a workplace located 175 Brampton Rd., Garden City, GA 31408.

29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures:  a)    At the Garden City warehouse: On or about April 27, 2017, and times prior thereto, the employer did not ensure that protective footwear was worn by forklift operators exchanging 33-lb. propane cylinders, exposing employees to struck-by, crush-by foot hazards.    Distribution Services International, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(1), which was contained in OSHA inspection number 951934, citation number 1, item number 3, and was affirmed as a final order on May 22, 2014, with respect to a workplace located 175 Brampton Rd., Garden City, GA 31408.
