Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly: a) Building A: On or about December 5, 2024, and at times prior to, the employer exposed employees to safety hazards, in that the Caterpillar-Yellow, Model#GC55K, Serial#AT88B10365 was equipped with a clamp attachment that was not approved by the manufacturer.

29 CFR 1910.178(a)(5):29 CFR 1910.178(a)(5): Powered industrial truck(s) equipped with front-end attachments other than factory installed attachments require the user to request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Building A: On or about December 5, 2024, and at times prior to, the employer exposed employees to safety hazards, in that there were no markings to identify the clamp attachment and show the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered, nor were the load charts updated on the Caterpillar-Yellow, Model#GC55K, Serial#AT88B10365 to reflect the bucket attachment.
