29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Fabrication Cell - On or about April 26, 2017 and times prior to, the drill guard for drill press DRL02 was inadequate in that the lower black portion and inner housing created a caught-in hazard for employees who used the drill to drill holes into copper tubing.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Battery Charging/Maintenance Area - On or about, April 26, 2017 and at times prior to, powered industrial truck (forklift) operators used an orange C&D Technologies Industrial Battery Charger that was not installed and used per the manufacturer's safe operating procedures, in that the battery charger was stored on combustible material such as a wooden pallet, thus exposing employees to a fire hazard. b) Battery Charging/Maintenance Area - On or about, April 26, 2017 and at times prior to, powered industrial truck (forklift) operators used a gray SCR Charger FLX Industrial Battery Charger that was not installed and used per the manufacturer's safe operating procedures, in that the battery charger was stored on combustible material such as a wooden pallet, thus exposing employees to a fire hazard. c) Laser Department Storage Rack Area - On or about, April 26, 2017 and at times prior to, powered industrial truck (forklift) operators used a black V-Force Battery Charger that was not installed and used per the manufacturer's safe operating procedures, in that the battery charger was stored on combustible material such as a wooden pallet, thus exposing employees to a fire hazard.
