29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) Building 2, Winding Machine 51: On or about April 28, 2017, and at times prior thereto, employees were exposed to a caught in hazard from while removing the shaft assembly that did not have specific procedures to eliminate the hydraulic pressure from the winding machine. (b) Building 2, Winding Machine 52: On or about April 28, 2017, and at times prior thereto, employees were exposed to a caught in hazard from while removing the shaft assembly that did not have specific procedures to eliminate the hydraulic pressure from the winding machine. (c) Building 2, Winding Machine 53: On or about April 28, 2017, and at times prior thereto, employees were exposed to a caught in hazard from while removing the shaft assembly that did not have specific procedures to eliminate the hydraulic pressure from the winding machine. (d) Building 2, Winding Machine 54: On or about April 28, 2017, and at times prior thereto, employees were exposed to a caught in hazard from while removing the shaft assembly that did not have specific procedures to eliminate the hydraulic pressure from the winding machine.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that the periodic inspections had been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: Building 2, Winding Machine 51, 52, 53, 54: On or about April 28, 2017, and at times prior thereto, the employees were exposed to the unexpected release of energy without having appropriate instructions on how to render the winding machines inoperative before performing any servicing or maintenance.
