On or about April 26, 2017, in plant 1, main press line at press 304, a supervisor who used the Toyota stand-up end-control lift truck #879 (model: 8BNCU20; S/N: 78067) to lift a steel A-frame hoisting structure dismounted the lift with the load engaging means in the raised position under the main steel beam of the structure.
