29 CFR 1910.217(b)(7)(ii): A red color stop control shall be provided with the clutch/brake control system. Momentary operation of the stop control shall immediately deactivate the clutch and apply the brake. The stop control shall override any other control, and reactuation of the clutch shall require use of the operating (tripping) means which has been selected.   a.  On or about March 22, 2018, in plant 1, department 1000, the emergency stop button for the 100 ton Federal press 301 (model: S2-100-48-42; S/N: S2-217) was not red in color.

29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.   a.  On or about March 22, 2018, in plant 1, department 1000, the point of operation at the exit end of the 100 ton Federal press 301 (model: S2-100-48-42; S/N: S2-217) was not guarded.

On or about March 22, 2018, in plant 1, department 1000, the two hand control station/stand for the 200 ton Bliss press 312 (model: SC2-200; S/N: H-71352) had an unused opening due to a missing knockout, thereby exposing employees to an electrical contact hazard.

29 CFR 1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.   a.  On or about March 22, 2018, in plant 1, department 1000, the flexible cord for the conveyor motor at the 100 ton Federal press 301 (model: S2-100-48-42; S/N: S2-217) was spliced and wrapped with tape, thereby exposing employees to an electrical contact hazard.
