29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 5/03/17, on the south side of a house at 580 Campbell Blvd., Getzville, NY: Employees used an extension ladder to access the roof at a point just above a roofing jack on the roof. The side rails of that ladder only extended approx. 22 inches above the roofing jack. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 5/03/17, on the roof on the south side of a house at 580 Campbell Blvd., Getzville, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 12 ft. No fall protection system was utilized. Pitch of the roof was approx. 12 in 12. NO ABATEMENT CERTIFICATION REQUIRED COLLINGWOOD CONSTRUCTION CORP. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1131190, CITATION NO. 1, ITEM NO. 2, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON 04/14/2016, WITH RESPECT TO A WORKPLACE LOCATED AT 8172 WOODVIEW COURT, WILLIAMSVILLE , NY 14226. COLLINGWOOD CONSTRUCTION CORP. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 550834, CITATION NO. 1, ITEM NO. 2, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON 10/22/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 690 DELAWARE AVENUE, BUFFALO, NY 14202.
