29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: (a) On or about 4/25/2017 - at NW Corner of Kelso Dr. and Citation Dr. in the Burgreen Farms Construction Project, an employee was not protected from the excavation face loose rock and soil when in a trench approximately 3.3 to 3.6 feet deep.
