29 CFR 1926.602(c)(1)(viii): Whenever a truck was equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, a safety platform firmly secured to the lifting carriage and/or forks was not used:    JOBSITE:  An employee was hoisted/lifted by the forks of the Gradall Material Handler to the upper level of a residential house under construction.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     JOBSITE:  The employees, engaged in residential framing activities were not protected from the hazard of falling between 10 feet 1 inch to 11 feet 5 inches to the ground by the use of appropriate fall protection guardrails systems, personal fall arrest systems or other equivalent means.    Strong Tower Construction LLC was previously cited for a violation of this Occupational Safety and Health standard {29 CFR 1926.501(b)(13)}, which was contained in OSHA Inspection Number 640238, Citation Number 01, Item Number 002a and was affirmed as a final order on 09/27/2012, with respect to a workplace located at 37 Narrow Rocks Road, Westport, CT 06880.
