The mixer has a minimum of electrical, hydraulic, pneumatic and gravitational energies. Employees were exposed to the unexpected start-up of the equipment and subsequently being caught-in the mixer.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: On or about May 9, 2017, at the vacuum mixer, the employer failed to ensure the employees were trained and they had the knowledge and skills for the safe application, usage and removal of energy control devices when cleaning/servicing of multiple energy source equipment. The vacuum mixer had electrical, hydraulic, pneumatic and gravitation energies. An employee was cleaning the mixer and the employee was exposed to the unexpected start-up of equipment and subsequently a caught-in hazard.
