On or about April 29, 2022, employees working in plastisol coating lines traverse over and around hoses that connect vats to the plastisol coating machines, exposing employees to a trip and fall hazards.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed. On or about April 29, 2022, the employer did not certify that periodic inspections were performed of energy control procedures on plastisol coating lines 1 and 2. Plastisol coating lines had equipment with multiple energy sources including electric, pneumatic (air) and mechanical, exposing employees to the hazards associated with unexpected startup.
