29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) Jobsite, 134 Riverside Drive, Ford City, PA: On or about May 9, 2017 employees engaged in steel erection activities approximately 35-45 feet above the ground were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or fall restraint systems.

29 CFR 1926.761(c)(3)(i): Where CDZs are being used, the employer did not assure that each employee has been provided training in the following areas: The nature of the hazards associated with work within a controlled decking zone; and the establishment, access, proper installation techniques and work practices required by � 1926.760(c) and � 1926.754(e): (a) Jobsite, 134 Riverside Drive, Ford City, PA: On or about May 9, 2017 the employer did not assure that each employee engaged in steel erection activities understood the requirements of when a controlled decking zone could be used and what forms of fall protection could be used. Employees were not aware of the height restrictions and were intending to use a CDZ in an area of the structure that was more than 30 feet above a lower level.
