29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about February 25, 2026, at a job site located at 20700 Harrisburg Westerville Rd., Alliance Ohio 44601; during the installation of roofing material (gutters) on a new construction commercial steel building, the employer did not ensure all employees using an aerial lift were protected from fall hazards varying in height of approximately 13 to 24.6 feet above the ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about February 25, 2026, at a job site located at 20700 Harrisburg Westerville Rd., Alliance Ohio 44601; during the installation of roofing material (gutter) at a new construction commercial steel building; the employer did not ensure employees working on the roof were protected from a fall hazard of 24.6 feet by utilizing a fall protection system. An employee accessed the roof to remove snow and ice so water would not drip on them as they installed gutters.
