(a) Location: bridge deck On or about 5/10/2017, employees were not protected from stepping into holes 3 to 4 inches deep and varying widths and lengths which were left uncovered after chipping out the concrete. (b) Location: between bridge deck and roadway On or about 5/10/17, employees were not protected from stepping into/through a 3 foot wide by 36 foot long opening made when the expansion joint was removed and the opening not covered.
