The employer did not ensure that a guardrail extended around the entire balcony edge/side where the Case Packer was located.     (b)  Employees using the stairway to access the office where exposed to a fall hazard of approximately 10 feet.  The employer did not ensure the upper landing of the stair way was properly guarded on all open sides.
