(a) On or about August 14, 2019, employees were installing new piping with personal fall protection systems and were using the fermenter tanks eyelets at the top of the tank for anchor points which are designed to hoist the tank and not for fall protection anchor points.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    (a) On or about August 15, 2019, employees servicing the glycol piping equipment and compressor on the equipment mezzanine above the shipping and receiving doors were not protected from falling to the lower level. This exposed employees to fall hazards approximately 14 feet in height.    Milwaukee Brewing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.28(b)(1)(i), which was contained in OSHA inspection number 1231632, citation number 1, item number 1 and was affirmed as a final order on 10/16/17, with respect to a workplace located at 613 South 2nd St, Milwaukee WI 53204.
