29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) Citation 1, Item 1a: Employee operating forklift had not received required training. b) Citation 1, Item 1b: Failed to provided written operator training certification. c) Citation 1, Item 2a: Employer did not develop and maintain a safety and health program to  provide frequent and regular safety jobsite inspections.  d) Citation 1, Item 2b: The employer failed to provide warning lines for employees exposed to fall heights greater than six feet. e) Citation 1, Item2c: Employer allowed employees to use ladder with damaged side rails. f) Citation 1, Item2d: Employer allowed employees to use a ladder that did not have slip resistant feet.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5) and as referenced by the note contained in 1926.602(d).  On or about May 10, 2017 at 130 W Franklin Avenue in Neenah, Wisconsin, an employee was operating a JBC 506 telescopic handler forklift for moving roofing materials on a site prior to receiving required operator training.

29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with accident prevention; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about May 10, 2017 at 130 W Franklin Avenue in Neenah, Wisconsin, the employer did not conduct adequate safety inspections prior to employees working on a roof surface 15 ft. above surface level.  No warning lines installed at roof access point, improper set-up of ladder, and damaged extension ladder not removed from service.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.  On May 10, 2017 at 130 W Franklin Avenue in Neenah, Wisconsin, employees were walking on a roof surface between two access points 15 ft. above surface level without any warning lines installed to guide the employees away from the unprotected roof edge.

On May 10, 2017 at 130 W Franklin Avenue in Neenah, Wisconsin, employees were using a damaged extension ladder to access/egress a roof surface at 15 ft. above surface level.  One side-rail was bent at the base of the ladder.

29 CFR 1926.1053(b)(7): Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.  On May 10, 2017 at 130 W Franklin Avenue in Neenah, Wisconsin, employees were using an extension ladder positioned on concrete surface to access/egress a roof surface at 15 ft. above surface level without using the slip resistant feet pads.  The ladder base feet were flipped with the metal tip on the concrete.
