29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. On or about May 10th, 2017, an employee working in the pasting area suffered a partial finger amputation while working with and cleaning her pasting machine. Her gloved hand became entangled with an unguarded chain and sprocket assembly as well as a rotating gear assembly. The original (partial) guard assembly over these hazard areas had been removed for unknown reasons before the accident happened.
