29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:    On or about May 10, 2017, at the job site employees were observed not wearing safety glasses or face shield while operating a circular saw.   Employees were exposed to eye or face hazards from flying particles.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    On or about May 10, 2017, at the job site an employee was observed working in an aerial lift approximately 18 feet above the ground.  The employee was not wearing a personal fall arrest harness and tying off to the boom or basket.  The employee was exposed to fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about May 10, 2017, at the job site an employee was observed working on the roof of a building approximately 20 feet high.  The employee was not wearing personal fall arrest system and had np other type of fall protection system provided.  The employee was exposed to fall hazards.
