29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: Abbyland Foods, Inc. facility located at 502 E. Linden St, Abbotsford, WI: On and about May 6, 2017, employees were exposed to amputation hazards while performing setup and servicing activities on the B.W Cooney & Associates Inc. tray wrapping machines. The specific procedures created for the tray wrapping machines did not specify the intended use of the procedure nor did give a method for testing or verifying deenergization that all authorized employees were qualified to perform.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Abbyland Foods, Inc. facility located at 502 E. Linden St. Abbotsford, WI: On and about May 6, 2017, the employer did not ensure employees utilized energy control application steps prior to them engaging in jam clearing or adjustment activities on the B.W. Cooney STN8500WER tray wrapping machines. The machine was not deenergized prior to the employee's engaging in the servicing/maintenance activity, nor were alternative protective measures implemented that provided effective protection in that the interlocked guard was not control reliable. As a result, the remaining applicable energy control elements, involving machine shutdown [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
