29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection: a) Navy Gateway Inn and Suites, Dam Neck: On or about 05/11/2017, the Honda generator EB6500 used to supply temporary power to a portable core drill was not provided with a ground fault circuit interrupter device thereby exposing employees to a possible shock hazard. No abatement certification or documentation required.

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) Navy Gateway Inn and Suites, Dam Neck: On or about 05/11/2017, employees worked in spaces that had wood boards covering holes greater than 2 inches in diameter in concrete floors and the wood boards were not secured. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
