29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.: a) Norfolk Federal Building, Atrium: On or about 11/05/2019, employees used an extension cord plugged into permanent power to supply power to a hammer drill, bore machine and water pump without a ground fault circuit interrupter device as a result employees were potentially exposed to a shock hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) Norfolk Federal Building, Atrium: On or about 11/05/2019, the wiring that entered into the switch box was not provided with stain relief in that the 90-degree flexible metal conduit connector was pulled away from the box, as a result employees were exposed to a potential electrical shock hazard without the protection of a GFCI device. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
