OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards.  a) Die Shop Area, on or about and at times prior to May 8, 2017, employees (machinist) were allowed to operate Computer Numerical Control (CNC) lathe machines, such as, but not limited to a (Model: Mazak Quick Turn Nexus 450-II/S/N: 240657) on a daily basis attaching work (billets) onto a Kitagawa (Model: B-21- CL1-041) B-Type chuck for lathing.  The gripping jaws attached to the chuck were not being used per the manufacturers safe operating instructions to prevent fly-out, exposing employees to struck-by hazards.
