29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. In the shop, the Ellis horizontal band saw used to cut metal tubing does not have a guard on the unused portion of the blade, exposing employees to an amputation hazard. The employer fails to assure effective protection against employee injury as required by the standard, as demonstrated on May 11, 2017, when the band saw was identified without a guard on the unused portions of the blade, exposing employees to an amputation hazard at 484 NW Turner Avenue, Lake City, Florida. TNT Metal Buildings was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1)), which was contained in OSHA inspection number 683859, citation number 1, item number 4 and was affirmed as a final order on December 6, 2012, with respect to a workplace located at 484 NW Turner Avenue, Lake City, Florida. TNT Metal Buildings was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1)), which was contained in OSHA inspection number 891418, citation number 1, item number 2 and was affirmed as a final order on April 5, 2013, with respect to a workplace located at 484 NW Turner Avenue, Lake City, Florida.

On offhand grinding machine work rests were not adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: a. In the shop, the Skil pedestal grinder does not have the work rest adjusted to a maximum of 1/8 inch opening: The employer fails to assure effective protection against employee injury as required by the standard, as demonstrated on May 11, 2017, when the grinder was identified as being used with the guard not adjusted correctly at 484 NW Turner Avenue, Lake City, Florida, exposing employees to a struck by hazard. TNT Metal Buildings was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.215(a)(4), which was contained in OSHA inspection number 683859, citation number 1, item number 5a and was affirmed as a final order on December 6, 2102, with respect to a workplace located at 484 NW Turner Avenue, Lake City, Florida.
