29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Worksite at 1025 W I-35 Frontage Rd. Edmond, OK 73034. On or about May 02, 2017 employees of a subcontractor working below the scaffolding were not provided with protective helmets, exposing the subcontractor's employees to struck-by hazards of falling objects.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Worksite at 1025 W I-35 Frontage Rd. Edmond, OK 73034 On or about May 02, 2017 employees of a subcontractor were working from scaffolding that was not provided with guard rails or other forms of fall protection, exposing the subcontractor's employees to a fall hazard of more than 10 feet.
