The blue Komatsu sit down forklift did not have functioning back-up lights or horn. b) Dock Area- On May 12, 2017, the employer did not remove the powered industrial vehicle from service when damage or unsafe conditions were identified. The orange Toyota sit down forklift had broken back-up light covers and was missing the latch that held the LPG tank in place.
