The bracket to secure the tank was missing.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):       a) Pallet Manufacturing Building- On May 12, 2017, the employer did not remove the powered industrial vehicle from service when damage or unsafe conditions were identified. The blue Komatsu sit down forklift did not have functioning back-up lights or horn.       b) Dock Area- On May 12, 2017, the employer did not remove the powered industrial vehicle from service when damage or unsafe conditions were identified. The orange Toyota sit down forklift had broken back-up light covers and was missing the latch that held the LPG tank in place.
