29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    The specific energy control procedure for the Stamtec 80T mechanical power press did not include the bar/stock straightener and coil feed machine, the magnitude of the hazardous energy sources on these machines, 220 Volt AC electrical and 80 p.s.i. for pneumatic, and the specific steps for isolating, locking out and verification of the bar/stock straightener and coil feeder machines.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    The employer had not conducted a periodic inspection of the energy control procedure on at least an annual basis.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    An employee who performed servicing and maintenance on bar stock straightener machines were not required to;    1. turn off and shut down the machine    2.  physically locate and operate the energy isolating devices, 220 Volt AC electrical energy source, in such a manner as to isolate the machine from the energy source.    3.  affix the lockout devices to the electrical energy isolating device    4.  prior to starting work on the machine that have been locked out, verify that isolation and deenergization of the machine had been accomplished.    On May 6, 2017, a maintenance employee was exposed to an amputation while oiling the chain and sprocket on the energized bar straightener machine using his thumb and fingers as a part of preventative maintenance which resulted in a partial amputation of the right thumb.
