29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    An employee who performed servicing and maintenance on bar stock straightener machines were not required to;    1. turn off and shut down the machine    2.  physically locate and operate the energy isolating devices, 220 Volt AC electrical energy source, in such a manner as to isolate the machine from the energy source.    3.  affix the lockout devices to the electrical energy isolating device    4.  prior to starting work on the machine that have been locked out, verify that isolation and deenergization of the machine had been accomplished.    On May 6, 2017, a maintenance employee was exposed to an amputation while oiling the chain and sprocket on the energized bar straightener machine using his thumb and fingers as a part of preventative maintenance which resulted in a partial amputation of the right thumb.
