Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   a) On or about June 5, 2020 the employer did not ensure that energy control procedures for the MOX Compound machines were documented and utilized by employees prior to allowing them to engage in troubleshooting, unjamming and set up activities.

29 CFR 1910.147(c)(7)(i)(A): Training and communication.  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   a) On or about June 5, 2020 the employer did not ensure that compound machines operators were trained to isolate energy sources and render equipment inoperative prior to allowing them to perform maintenance and set up activities on the three compound machines.

29 CFR 1910.212(a)(1):  Machine Guarding: Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) On or about June 10, 2020 the MOX Compound machine did not utilize a guarding method that would prevent employees from getting caught in the exchange mechanism where the rings are stacked.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  Employees were not required to:  (1) turn off and shut down the machine (2) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (3) affix lockout or tagout devices to each energy isolating device (4) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.    a) On or about June 5, 2020 on the MOX compound machine, employees, such as operators performing troubleshooting, and cleaning of the needle did not lockout the electrical and pneumatic disconnects when making part adjustments within the guard of the machine.  b) On or about June 10, 2020 on the MOX, MO7 and MO8 Compound machines employees, such as operators performing servicing of the needle did not lockout the electrical and pneumatic disconnects when cleaning the needle within the guard of the machine.    The West Troy, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.147(d), which was contained in OSHA inspection number 1232427, citation number 1, item number 3 and was affirmed as a final order on June 28, 2017, with respect to a workplace located at 650 Olympic Dr. Troy, Ohio 45373.
