29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternatives fall protection measures:  JOB SITE: 2711 Towne Oaks Drive, Saint Louis, MO 63129, on or about 5/12/2017, workers were observed on top of a 4:12 pitch roof, approximately 9' 7" eave to ground level, actively installing new roofing shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system nor were they provided with an alternatives fall protection measures.  The workers were exposed to fall hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  JOB SITE: 2711 Towne Oak Drive, Saint Louis, MO 63129, on or about 5/12/2017, workers were observed using a Werner Aluminum Extension Ladder, MODEL: D1224-2, 225 lb capacity, to gain access on and off the roof surface and the ladder's side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.

The workers were exposed to a fall hazard.
