29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface.  The employer did not ensure that the warning line was erected so its lowest point was not less than 39 inches from the walking/working surface of the roof when:  The warning line system on the back side of the roof was sagging so the warning line was approximately twelve (12) inches from the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    The employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a flat roof that was less than 50 feet wide.  The employees were exposed to fall hazards of approximately fourteen (14) feet.  United Construction Solutions, LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1232472, citation number 1, item number 1 and was affirmed as a final order on 07/10/2017, with respect to a workplace located at 2711 Towne Oaks, Saint Louis, MO 63129.  United Construction Solutions was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1004327, citation number 1, item number 3 and was affirmed as a final order on 01/06/2015, with respect to a workplace located at 217 Sycamore, Fulton, MO 65251.
