On or about 5/16/2017 at buildings 300 at 315 Oconee Street, Athens, GA: Employer failed to ensure frequent and regular inspections were conducted of the site area by a competent person designated by the employer, to include all material and equipment such as but not limited wood planks used as work platforms, guardrails systems, debris on work platforms, and safe access and egress on all of the mast climbing and metal fabricated scaffolding systems being used by the employees.
