29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: At the jobsite, an excavation greater than 5 feet in depth with standing water was not inspected by a competent person; exposing the employees to cave-in hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite, two employees were performing sewer line connections in an excavation with water and greater than 5 feet deep without a protection system in place, and were exposed to cave-in hazards. Texas Pride Utilities Limited Liability Company was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA inspection number 1099869, citation number 1, item number 3 and was affirmed as a final order on 12/11/2015, with respect to a workplace located at the intersection of 4000 7th St and Thompson Rd, Bay City, TX 77414.
