At or about May 19, 2017 and times prior thereto at the worksite located at or near 3321 S Bowman Rd in Little Rock, Arkansas, employees were observed operating pneumatic nailguns while wearing sunglasses. This condition exposed employees to struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At or about May 19, 2017 and times prior thereto at the worksite located at or near 3321 S. Bowman Road in Little Rock, Arkansas, employees were observed laying floor-boards on the top of new construction without any means of fall protection. This condition exposed employees to fall from heights hazards in the following instances: Instance A: Employees laying floor-boards on the apartments located on the north side of the construction project. Instance B: Employees laying floor-boards on the apartments located south of the apartments in Instance A. Instance C: Employees preforming edge work on the apartments located on the north side of the construction project.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. At or about May 19, 2017 and times prior thereto at the worksite located at or near 3321 S Bowman Rd in Little Rock, Arkansas, employees were accessing the top of the construction with a portable ladder that did not extend 3 feet beyond the upper level. This condition exposed employees to fall from height hazards.
