29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about May 19, 2017, the employers did not protect employees from falls on walking-working surfaces with unprotected sides or edges that were 4 feet or more above a lower level.  Employees were exposed to fall hazards.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a) On or about May 19, 2017, the employer did not ensure employees working at heights were properly trained in the use of personal fall protection systems as required.  Employees were exposed to fall hazards.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:  a) On or about May 19, 2017, the employer did not ensure a period inspection review of the energy control lockout procedures were conducted between the inspector and each authorized employee.  Employees were exposed to electrical hazards.
