29 CFR 1910.22(b): For every walking and working surface used for storage, the employer did not ensure that the walking and working surface could support the maximum intended load for that surface: a) On or about March, 2017 in the custodial closet of the Nancy Hanks Lincoln Hall, the custodial/maintenance employees entered the pipe alley by standing on the sewer pipe. It had not been determined if the piping was capable of supporting the employees.

29 CFR 1910.146(k)(1)(ii): The employer did not evaluate a prospective rescue service's ability, in terms of proficiency with rescue related tasks and equipment, to function appropriately while rescuing entrants from the particular permit space or types of permit spaces identified: a) On or about March, 2017 in the custodial closet of the Nancy Hanks Lincoln Hall, the custodial/maintenance employees were entering the pipe alley, which is a permit required space. The employer's rescue service, the Santa Claus Volunteer Fire Department, was not trained to rescue employees from the permit required confined spaces and would contact another fire department for these emergency calls.
