A.) All-Weather Roofing Company, Inc., 3917 Station Road, Erie, PA 16510, on or about May 10, 2017 and times prior thereto: Employees were exposed to a fall hazard of 6.4-feet and 18.9-feet when performing roofing activities that included, but were not limited to, the removal of roofing materials. Employees were not utilizing a means or methods of fall protection.

All-Weather Roofing Company, Inc., 3917 Station Road, Erie, PA 16510, on or about May 10, 2017 and times prior thereto: Employees were utilizing electrical power tools including, but not limited to, a circular saw to perform roofing activities. The employees were not utilizing a GFCI when utilizing the electrical power tools. The company does not have an assured equipment grounding conductor program
