29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Foyer Roof - Employees, wearing personal fall arrest equipment, installed the Foyer roof, and when spraying the adhesive to apply the insulation board and while installing the insulation board, did not tie off or use other fall protection, exposing the employees to the hazard of falling 6.5 feet to the lower roof on the east, south, and west sides of the Foyer roof, 12 feet 4 inches to the Porch roof on the north east side of the Foyer roof, and 23 feet to the ground on the north west side of the Foyer roof, on or about May 18, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
