29 CFR 1926.451(g)(4)(iii):When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform: (a) MANHEIM TOWNSHIP MIDDLE SCHOOL, NEFFSVILLE, PA, ENTRYWAY ROOF - Three employees used a JLG 4394RT Scissor Lift to gain access to an entryway roof, and the mid-chain on the access doors were not fastened, which exposed the employees to a 13 to 14 foot fall to the ground below, on or about April 6. 2021. No abatement certification or documentation required.

No abatement certification or documentation required.

No abatement certification or documentation required.
