29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. Jim Palmer Excavating, Inc. worksite located at 434 7th St., Perrysburg, Ohio: On or about May 22, 2017, the employer did not ensure employees were adequately protected from cave-ins while installing sanitary lines on a residential construction site. Employees were working in an unprotected trench approximately 8.50 feet deep consisting of primarily B soil with near vertical walls.
