A) Jim Palmer Excavating, Inc. on or about March 26, 2019 at the intersection of Birch Drive and Highland Drive in Rossford, OH: The employer did not ensure employees were protected from head injuries while working in a 9 foot deep excavation. Employees were exposed to impact injuries caused from falling materials and/or equipment such as the unsupported excavation wall and the excavator arm overhead.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). A) Jim Palmer Excavating, Inc. on or about March 26, 2019, at the intersection of Birch Drive and Highland Drive in Rossford, Ohio: The employer failed to ensure employees were adequately protected from cave-ins when working in a 9 foot deep excavation, while leveling a manhole and manually digging around the manhole tie-ins. Jim Palmer Excavating was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1234807, citation number 1, item number 1 and was affirmed as a final order on June 20, 2017, with respect to a workplace located at 434 7th Street, Perrysburg, Ohio.
