29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about May 25, 2017, during roofing activities including, but not limited to, hoisting and removing roofing materials and equipment, employees were not protected from a fall of approximately 20 feet  by utilizing a guardrail system, fall protection system, and/or combination of warning line system and safety monitor system.  A fall protection plan was not developed or implemented and fall protection was not utilized during the periods of installation of roofing material occurring between the front gables of the house.   American Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 907945, citation number 01, item number 01 and was affirmed as a final order on January 07, 2014, with respect to a workplace located at 24434 Lake Road Bay Village, OH 44140.  American Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315622498, citation number 01, item number 01 and was affirmed as a final order on October 25, 2012, with respect to a workplace located at 30336 Lake Road, Bay Village, OH 44140.
