Employees are operating powered industrial trucks with defects. On and about 05/18/17 employees operated forklifts with missing slip resistant pads on brake pedals, exposing other employees to struck-by hazards. .

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: 3144 N Broadway: Employer is exposing employees to hazardous conditions. The employer is exposing employees to amputation hazards where the guard of a floor fan with metal blades has openings which are larger than 1/2 inch. On and about May 18, 2017 the employees were exposed to openings which were measured at approximately 4 inches on the guard of a floor fan with metal blades periphery which was less than 3 feet from workplace floor.
