The employer is exposing employees to amputation hazards where the guard of a floor fan with metal blades has openings which are larger than 1/2 inch.      On and about May 18, 2017 the employees were exposed to openings which were measured at approximately 4 inches on the guard of a floor fan with metal blades periphery which was less than 3 feet from workplace floor.
