29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about 24 May 2017 and at times prior thereto, two employees were using pneumatic nail guns to apply roofing shingles to the roof of a residence under construction without the use of eye protection, exposing the workers to the hazard of being struck in the eyes by flying nails and debris.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about 24 May 2017 and at times prior thereto, 3 workers were performing roofing activities on the roof of a residence under construction without fall protection, exposing the workers to the hazard of falling approximately 15 feet to the ground below.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about 24 May 2017, and at times prior thereto, three roofers accessed the roof of a residence under construction using an extension ladder that was not extended at least 3 feet above the upper landing, exposing the roofers to a fall hazard of approximately 9 feet to the ground below.
