Crown Equipment Corporation located in New Bremen, Ohio: On or about May 22, 2017 the employer did not ensure employees using a coolant hose to spray metal chips from the cutting area of the Amada HFA 400W horizontal bandsaw, were adequately trained to lock out the saw and material clamp prior to performing the cleaning operation.
