OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:       On or about May 24, 2017 and times prior thereto, at the worksite located at or near 190 Wilson Mill Road in Monticello, Arkansas, the employer failed to ensure that employees were wearing the available seatbelts while operating forklift trucks. Employees were exposed  to ejection and crushed-by hazards.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.    On or about May 24, 2017 and times prior thereto, at the worksite located at or near 190 Wilson Mill Road in Monticello, Arkansas, the exit routes located at the end of the knot-saw production areas were not at least 28 inches wide at all points. This condition exposed employees to delay in exit and required travel through a high hazard area.

29 CFR 1910.95(b)(1):  When employees are subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls shall be utilized. If such controls fail to reduce sound levels within the levels of Table G-16, personal protective equipment shall be provided and used to reduce sound levels within the levels of the table.    On or about May 24, 2017 and times prior thereto, at the worksite located at or near 190 Wilson Mill Road in Monticello, Arkansas, employees in the stacker area were exposed to sound levels in excess of 97.3dBA were not required to wear any hearing protection. This condition exposed employees to noise hazards.

On or about May 24, 2017 at the worksite located at or near 190 Wilson Mill Road in Monticello, Arkansas, the circuit breakers located on the second floor of the kilns were not labeled with voltage, current, wattage, and other ratings as required. This condition exposes employees to shock hazards.
