On or about July 20, 2021 and times prior thereto, at the knot saws, employees were not provided a safe means of access and egress in the area where the knot saws were located including, but not limited to, climbing up or down a 30-inch step to maneuver over the conveyor.

This condition exposed employees to trip and fall hazards in the event of an emergency evacuation.

The MAXWELL HARDWOOD FLOORING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(r)(4), which was contained in OSHA inspection number 1235423, citation number 1, item number 6 and was affirmed as a final order on 11/06/2017, with respect to a workplace located at 190 Wilson Mill Road in Monticello, AR.
