29 CFR 1910.67(c)(2)(v): The employer did not ensure a personal fall arrest or travel restraint system that meets the requirements in subpart I was worn and attached to the boom or basket when working from an aerial lift: An employee was exposed to a fall hazard of approximately 11 feet while operating a vehicle mounted aerial lift to mount a light to a tree and a a personal fall arrest or travel restraint system was not used. S & S Lighting Maintenance, Co. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.67(c)(2)(v), which was contained in OSHA inspection number 1051953, citation number 1, item number 1a and was affirmed as a final order on 05/15/2015, with respect to a workplace located at 113 East Gate Drive, Washington, Illinois, 61571.
